Someone please take a look at this diagram and tell me where to hook the 12V and the 5V (which pins)

I'm trying to use it to turn on and off my fan with the IO. I already have a 150ohm 10W resister bringing it down to 12v from 19v. All I need it the relay hooked up and I'm golden!

To answer my own post:

Hey stupid! Where's the link to the diagram!? DOH! No worries... asked a friend who works in our Roadshop; he helped my figure it out.
